The Grand River Community Health Centre (GRCHC) is providing accessible, inclusive non-urgent health services through its Primary Care Access Clinic, available at our centre.

This Clinic is designed to support individuals who do not currently have access to primary health care. Each clinic is staffed by a dedicated team of professionals committed to providing comprehensive, person-centered care.

Clinic Team:

• Physician
• Registered Nurse
• Outreach and Service Navigation Worker
• Registered Psychotherapist (Qualifying)
• Peer Support Worker
• Midwife

Services Provided:

• Preventative health care

• Cancer screening

• Pregnancy support

• Well-baby and well-child visits

• Vaccinations

• Sexual health services

• Smoking cessation support

• Counselling

We also provide care and support for non-urgent or chronic health conditions.
Acute illnesses (such as ear infections, sore throat, cough, or urinary tract infections) may be seen based on provider availability.

• This clinic is here to help anyone who doesn't currently have a primary care provider

Who Is Not Eligible

• Those who have an existing primary care provider (doctor or nurse practitioner)
