(formally known as Memory and Geriatric clinic)

About our program:

The Seniors Assessment Service at GRCHC operates using a MINT Memory Clinic-based model with enhanced care of the elderly/ geriatric expertise. We work collaboratively with the Regional Geriatric Program, Branford Brant Norfolk OHT, Brant Community Healthcare System, and the Alzheimer’s society of Brant

  • The program is referral- based. Referrals are triaged according to issues identified by the referring physician or primary care provider
  • Clinics currently run on Wednesdays and Thursdays.

Common reasons for referrals include:

  • Cognitive changes
  • Mobility changes, falls, frailty
  • Polypharmacy
  • Behaviours related to dementia
  • Caregiver burden

What to expect:

Caregivers have the opportunity to discuss concerns with staff privately before the assessment.
Assessment in clinic can take up to 2h hours and includes:

  • Cognitive testing
  • Assessment by nurse and physician
  • Medication review
  • Referral to services based on issues identified in the visit
  • Assessment regarding fitness to drive is done if cognition/ memory is reason for referral

Consult (one time) appointment, although follow ups are provided on an as needed basis. 

We aim to provide in the consultation diagnosis on cognitive changes if applicable, medication and lifestyle suggestions to slow cognitive decline, suggestions around medications causing negative impact on health, educational resources, and referrals tailored to the individual.

Eligibility:

  • Must have a Primary Care Provider to follow up on recommendations from the clinic
  • Generally 65+ years old, younger patients are reviewed and accepted on a case-by-case basis
  • Must live in Brantford/ Brant County/ Norfolk 

How to be referred:

On the day of your appointment:

What to bring: 

  • All medications (including vitamins, minerals)
  • Gait aid if using regularly
  • Glasses
  • Hearing aids
