Prescription Medication Cost Support – GreenShield Cares’ EMP
We have teamed up with GreenShield Cares’ Essential Medicines Program (EMP) to help provide Ontarians with Medication cost support. The program provides participants with up to $1,000 for a 12-month period towards eligible prescription medications.
Eligibility Requirements:

Canadian citizen or permanent resident of Ontario
-
Aged 25 to 64
-
Employed
-
Have a combined household net income below the program threshold
-
Not enrolled in a private drug plan, regardless of the level of coverage available

Not enrolled in a provincial drug program

Healthcare providers can download and complete the GreenShield referral form below and fax it to GRCHC to refer eligible patients to the program.
