Chronic Disease Management

Ontario Health has introduced Interprofessional Primary Care Teams (IPCT) to connect more people to primary care.  We are providing chronic disease education to compliment the care you are receiving from your primary care provider.

The interdisciplinary team delivers group education sessions on a variety of topics, such as diabetes, health health, brain health, and the power of protein.

The team also offers 1:1 appointments, such as:

    • Nutrition Counselling – Nutritional education for client across the human lifespan and guidance around goal setting to improve health. 
    • Mental Health Counselling – Supporting individuals with developing positive coping strategies and achieving overall mental wellness.
    • Social Prescribing – Assisting clients with connecting to non-clinical resources and services.
    • Diabetes Program – Helping individuals gain the knowledge and skills to effectively self-manage type 2 diabetes and providing recommendations to the referring provider when required.
