Chronic Disease Management

• The team will deliver education sessions across Brantford, Brant and Norfolk counties.
• Topics could include heart health, factors that influence heart health, preventing diabetes, living with diabetes, improving mental health and much more.
• There are also 1:1 offerings such as:
• Nutrition Counselling – Nutritional education for client across the human lifespan and guidance around goal setting to improve health.
• Mental Health Counselling – Supporting individuals with developing positive coping strategies and achieving overall mental wellness.
• Social Prescribing – Assisting clients with connecting to non-clinical resources and services.

This team will work together to provide a range of services to clients who do not have primary health care or whose primary care provider does not have services to support their clients.

The team is comprised of:

Registered Dietitian

Registered Nurse

Community Navigator/Social Prescriber

Everyone is welcome, whether you have a primary care provider (i.e., doctor or nurse practitioner) or not.
