- The team will deliver education sessions across Brantford, Brant and Norfolk counties.
- Topics could include heart health, factors that influence heart health, preventing diabetes, living with diabetes, improving mental health and much more.
- There are also 1:1 offerings such as:
- Nutrition Counselling – Nutritional education for client across the human lifespan and guidance around goal setting to improve health.
- Mental Health Counselling – Supporting individuals with developing positive coping strategies and achieving overall mental wellness.
- Social Prescribing – Assisting clients with connecting to non-clinical resources and services.

Ontario Health has introduced Interprofessional Primary Care Teams (IPCT) to connect more people to primary care. We are providing chronic disease education to compliment the care you are receiving from your primary care provider.

About the Interprofessional Primary Care Team (IPCT)
This team will work together to provide a range of services to clients who do not have primary health care or whose primary care provider does not have services to support their clients.
The team is comprised of:
Registered Dietitian
Registered Nurse
Community Navigator/Social Prescriber
Everyone is welcome, whether you have a primary care provider (i.e., doctor or nurse practitioner) or not.
*The program is not intended for people that have extended health benefits or have third party coverage.
