Diabetes Program

Ontario Health has introduced Interprofessional Primary Care Teams (IPCT) to connect more people to primary care.  We are providing chronic disease education to compliment the care you are receiving from your primary care provider.

Our Diabetes Self-Management Program is a free, supportive program designed to help community members living with diabetes feel confident and informed in managing their health. In addition to group education, participants have access to one-on-one appointments with a Certified Diabetes Educator(s), offering personalized guidance tailored to individual needs and goals. Educators work with participants to address challenges, answer questions, and develop realistic self-management strategies.

Led by knowledgeable health professionals, the program provides a welcoming space to learn, connect, and build confidence—whether you are newly diagnosed or have been living with diabetes for years.

To book an appointment or register for a class you can complete a self-referral form below or have your primary care provider refer you to our program.  

Pre-diabetes workshops:

Everyone is welcome, whether you have a primary care provider (i.e., doctor or nurse practitioner) or not.

Pre-diabetes is when your blood glucose levels are elevated, however they are not high enough to be diagnosed with Type 2 Diabetes. This is the time that lifestyle changes can play an important role. These lifestyle changes include diet, physical activity, and quitting smoking if applicable. Educating yourself about pre-diabetes and how to decrease your risk, or delay the onset, of developing Type 2 Diabetes can be done at the Pre-Diabetes Workshop at Grand River Community Health Centre. Reaching out to a Registered Dietitian is also a great place to start.

Type 2 Diabetes is a chronic health condition that occurs when your blood glucose levels are elevated. This can be due to your body not producing enough insulin, or not using the insulin it does produce properly. Positive lifestyle changes and self-management education are very important when diagnosed with Type 2 Diabetes, as both can help decrease your risk of complications.  Having a diagnosis of Type 2 Diabetes can feel overwhelming at times, and the health care staff at Grand River Community Health Centre are here to help. We can provide you with the knowledge and tools you need to self-manage your diabetes. The team at Grand River CHC consists of a Registered Nurse and Registered Dietitians who will meet you where you’re at, and help guide you along your journey.
