About the Clinic
The Gender Affirming Clinic at Grand River Community Health Centre offers services to individuals (adults 18+) with regard to issues related to gender identity and expression, including gender expansive, trans and non-binary identities.
Consultation and support may include:
- Prescribing transition-related hormones
- Diagnostic assessments in support of affirming a diagnosis of Gender Dysphoria in support of transition related surgeries
- Referrals for transition-related surgeries
How to Register
The Grand River Community Health Centre’s service catchment area is generally comprised of the City of Brantford, the County of Brant, and the County of Norfolk.
The Gender Affirming Clinic is at capacity and clients from outside of this area cannot be accommodated, at this time.
For appointments, please complete our Gender Affirming Clinic Intake form and return it to us by mail, fax or in-person.
