ACCEPTING NEW PATIENTS
If you do not currently have a Family Doctor or Nurse Practitioner and are interested in joining Grand River Community Health Centre (GRCHC), we would be happy to help you get started.
How to Register
Please complete our New Client Intake Form and drop it to reception during our operating hours. If you prefer a paper copy, you are welcome to visit the Centre and ask our reception team for a New Client Intake Package.

You can also register with Health Care Connect by calling 1-800-445-1822 or registering online through — https://hcc3.hcc.moh.gov.on.ca
Completing both the GRCHC intake form and Health Care Connect registration may help you connect with a Family Doctor or Nurse Practitioner faster.
